Our supported living services are tailored to meet the unique needs of each resident. With 24-hour on-site staffing, including waking night and sleep-in staff, we ensure round-the-clock care and supervision. Residents benefit from personalised care plans, assistance with daily tasks, and support in managing finances and medication. We also encourage participation in community activities, helping individuals build social connections and maintain a fulfilling lifestyle. For those ready to transition to greater independence, we offer life skills training, such as cookery lessons and budgeting support. Our approach is all about empowering residents to live confidently and independently.

We adopt a Person-Centred Planning approach that addresses the specific needs of residents and employs staff who are committed to meeting the following aims and objectives:

Supporting residents in promoting self-esteem through a structured, individualised enablement plan that recognises their abilities and encourages participation.
Reviewing the agreed plan of care regularly with residents and significant individuals.
Maintaining effective communication with residents and their representatives.
Ensuring the health and safety of residents at all times.

Eminent Care Agency provides accommodation for residents with learning disabilities, physical disabilities, and autism. We accommodate both young and older adults with similar needs.

Prospective residents will be assessed to determine if they require 24-hour staff presence.

We accept emergency placements as we have the resources and capacity to respond to such situations when needed.

All referrals are assessed before acceptance, and an assessment report, along with initial recommendations, will be made available to the referring professional or family member within 72 hours of the assessment.

We ensure that a programme of care is developed through individual needs assessments and evaluations. Residents will be supported in expressing how they want to be cared for.

Each resident will be assigned a key worker who coordinates their support and ensures that it is provided in an effective and constructive manner.

We provide social and emotional support by encouraging our residents to maintain links with leisure establishments and other community services.

Staff provide support and assistance with cooking and other domestic chores.

We develop a ‘Moving On’ plan for individuals ready to transition to a lower level of support by offering cookery lessons and other life skills that facilitate their move toward independent living.

When residents are taking medication, staff will prompt and assist them in following the prescriber’s instructions while maintaining close contact with relevant health professionals. If residents require assistance with managing their finances or administrative matters, staff will provide the necessary support as required.

We provide 24-hour on-site staffing. At night, there are two staff members on duty: one waking night staff and one sleep-in staff. Staff on duty will assist, supervise, and support residents in accordance with their identified needs. We tailor staffing levels to meet the needs of our residents.

In addition to the Care Manager and staff on duty, additional support is provided by the On-Call Manager.

Charges are based on full accommodation and the provision of support on a needs-led basis, which will be determined during the initial assessment.

Our fees are negotiable but depend on the assessed care needs and shared support hours. Residents who require additional support will receive 1:1 or 2:1 assistance, which will be costed accordingly.
